OD is just generally, a frustrating mode, and people that play it are somewhat masochist, myself included.
As many have said, the basic foundation of success has a lot to do with captain/coach combos, knowing the tactics, having a basic play script, and being able to manage your anger. From there, it is knowing how defenses line up, react when you hike the ball, and your receivers will behave in certain scenario. The most advance players will know how to make adjustments on the fly when certain tactics are called.
I've been in the top 500 multiple times, but will curse and moan at EA for this effing mode. I absolutely hate this mode and only play it because I need whistles and the rewards are just too good to give up. Go figure, the most "optional" mode in this game has the best rewards.
